Joan Therese Seivert and Connections Unlimited, LLC can be a valuable partner for your family, as an eldercare advisor and specialist, navigating through a host of senior resources.  Joan Therese provides you with a Senior Assessment, gives you referrals and helps with an in home assessment of the whole person and their situation. I give you referrals and help you identify the right  in home care services and  various professionals that maybe needed. I am also a placement service and can help with placement into retirement living communities, assisted living, memory care and nursing homes.  I maintain a current listing of available openings at all times.

Connections Unlimited's Preferred Provider Network member categories include:
In-Home Care Service Options 
From hourly to live-in help cooking, cleaning, laundry, medications, driving, shopping, dressing and bathing.

Assisted Living Placement
Large community or small residence setting - private room, activities, location, levels of care, and cost.

Continuum of Care 
Campus setting, variety of care levels, no more moving, some include medicaid, consideration for waiting lists.  See 2013 Continuum of Care here.


Memory Care 
Memory care, secured setting, structured activities and specialized staff training, Alzheimer's, large or small setting.


Nursing Home
24-hour nursing care and two-person assist.

 

Palliative & Hospice Care
Get the best referral option for in-home care services, products such as monitoring devices and other Denver community services that help older adults age in place.

Professionals
Including Elder Law Attorneys, Care Managers, Fiduciary Services, Doctors, Licensed Clinical Social Workers, Health Insurance Specialists, Long-term Care Insurance, Financial Planners, Mediators, and more...
